A form that you’d fill out at the doctor’s office is a טֹפֶס  listen and repeat in Hebrew. It comes from the Greek word τυπος (tupos) meaning something recorded, labeled, the same origin of the English word type.

In the plural, it’s טְפָסִים  listen and repeat. And in the plural when connected to another noun, it’s טוֹפְסֵי  listen and repeat, for example:
 
צָרִיךְ לְמַלֵּא אֶת טוֹפְסֵי הַהַרְשָׁמָה.
We need (one needs) to fill out the registration forms.
 
or simply:
 
צָרִיךְ לְמַלֵּא אֶת הַטְּפָסִים.
We need to fill out the forms.
 
 
Translation 2
 
While טופס is a form when talking about paperwork, there’s the other meaning of form, the more abstract one that refers to the shape or the appearance of something. That word is צוּרָה  listen and repeat.

For example:
 
לֵךְ עַד הַבִּנְיָן בְּצוּרָה שֶׁל עִגּוּל.
Walk until the building in the form of a circle.
 
or, more concisely:
 
לֵךְ עַד הַבִּנְיָן בְּצוּרַת עִגּוּל.
Walk until the building in a circle form.
